Violation 703: Emergency coordinator information lacking. <br /> Corrective action: 1) The "Emergency Notification" list of the Emergency <br /> action/evacuation plan has been revised to clarify <br /> Emergency coordinators action and identification. <br /> 2) An "emergency map" has been made part of the posting <br /> to identify locations of fire extinguishers and spill <br /> control equipment. <br /> 3) Attached is copy of posting with the "Emergency Map" <br /> 4) Emergency information posted by phones <br /> Minors: <br /> Violation 611: 55 gallon drums contained used oil filters over the one year <br /> start date. <br /> Corrective action: 1) Oil filters removed (Bill of lading attached) <br /> 2) Will be monitored on in-house inspection <br /> 3) Will seek assistance from waste hauler <br /> Violation 809: a Universal waste over one year. <br /> Corrective action: 1) accumulated universal waste disposed of ; See attached <br /> disposal documentation. <br /> Notes section of Inspection report. ( These items were corrected of time of <br /> visit.) <br /> 1) Manifests missing & manifest not sent to [)TSC ; <br /> Corrective action- Improved manifest record keeping was established <br /> to prevent further missing manifest. Investigating <br /> possibility of "milk run" service to eliminate need for DTSC mailing <br /> of manifest for used oil. <br /> 2) Used fluorescent bulbs were improperly stored. <br /> Corrective action: 1) Bulbs were immediately stored properly. <br /> 2) Training Meetings were held to educate employees. <br /> 3) Spill pallet under used oil containers had some oil in reservoir <br /> Corrective action: 1) Oil was removed at time of audit. <br /> 2) employee meeting held to educate <br /> 3) Will be monitored by in-house inspections <br /> 4) E-Waste and some universal waste was not labeled <br /> Corrective action: 1) universal waste labeled at time of audit <br /> 2) Area for a-waste was set up and marked <br /> 3) Employee meeting held for proper a-waste disposal <br /> and location of a-waste accumulation area. <br />
