The monitoring plan on site are not current. The monitoring plan must be current and approved by <br /> the EHD. Complete and submit a copy of the monitoring and plan for approval by the EHD within 30 <br /> days of receiving the report. An approved copy will be returned for your records. <br /> 201. The following alarms were either not addressed and /or not corrected: L4-2/9/11, L4-1/20/11, <br /> L7-1/10/11, L19-12/27/10, L2-3/18/11, L19-3/20/11, S9-3/22/11, L2-5/18/11, S7-5/21/11, L2-3/24/11, <br /> S5-4/22/11, S7-4/26/11, S9-4/7/11, and L10-10/25/11 . Records for all alarms and how they were <br /> corrected shall be kept on site for at least 3 years. Immediately locate and maintain the alarm logs on <br /> site and submit copies to the EHD within 30 days of receiving this report. <br /> 302. The Diesel fill sump liquid sensor failed functionality test and is not working properly to ensure <br /> continuous monitoring of the tank system as required by CCR 2630(d). Correct immediately by having a <br /> properly licensed, trained, and certified contractor replace the failed component with a functional <br /> component (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. <br /> The failed sensor was replace, retested and passed (Model 794380-208, SN 884386). A permit must be <br /> obtained within the next business day. <br /> 313. The 91 spill container failed when tested with a Caldwell tester. Drain valve and fill cap was <br /> changed on the 87 spill bucket to pass the test All spill containers shall have a minimum capacity of five <br /> gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br /> contact a properly licensed, trained, and certified contractor to repair or replace the spill container under <br /> permit and inspection of the EHD. A new spill container was installed on the 91 fill sump, retested and <br /> passed. Permit must be obtained the next business day. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector Received By Title. <br /> EHD 23-02 Rev 09/01/11 Page 3 UST INSPECTION REPORT <br />
