During the record review following observation was made on the 87 STP sump: 10/7/11 - 45 gallons of liquid was found and removed by Walton Engineering, 6/8/11 - 55 gallons of water was found and removed by LC Services, 4/7/11 - 90 gallons of water was found and removed by LC Services, 3/22/11 - 80 gallons of water was found and removed by LC Services, 2/25/11 - 180 gallons of water was removed from the 87 STP and Fill sump by LC Services, 11/9/10 - 35 gallons of liquid was found and removed by Walton Engineering. Today 7.5 gallons of liquid was found in the 87 fill sump, 4 oz of liquid was found in 91 fill sump, Diesel STP sump and the 7/8 Dispenser UDC indicating a leak in the secondary containment. Secondary containment shall be impervious to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of contact with any hazardous substances released from the primary containment. Immediately contact a properly licensed, trained, and certified contractor to make repairs to the UST system under permit and inspection of the EHD. The technician removed all liquid and put it in the facility's hazardous waste drum. 406. The Diesel STP double wall Sump sensor and the Diesel fill sump liquid sensor failed when tested for functionality. The sensors monitoring the liquid level in the interstitial space shall be maintained so that a breach in the primary or secondary containment is detected before the hazardous substance is released into the environment. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed component with a functional component (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. The failed sensor was replace, retested and passed (Model 794390-304). Permit must be obtained on the next business day. Notes Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, to the EHD within 30 days of receiving this report. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
