During the record review following observation was made on the 87 STP sump: <br /> 10/7/11 - 45 gallons of liquid was found and removed by Walton Engineering, <br /> 6/8/11 - 55 gallons of water was found and removed by LC Services, <br /> 4/7/11 - 90 gallons of water was found and removed by LC Services, <br /> 3/22/11 - 80 gallons of water was found and removed by LC Services, <br /> 2/25/11 - 180 gallons of water was removed from the 87 STP and Fill sump by LC Services, <br /> 11/9/10 - 35 gallons of liquid was found and removed by Walton Engineering. <br /> Today 7.5 gallons of liquid was found in the 87 fill sump, 4 oz of liquid was found in 91 fill sump, Diesel <br /> STP sump and the 7/8 Dispenser UDC indicating a leak in the secondary containment. Secondary <br /> containment shall be impervious to the liquid and vapor of the substance contained and constructed to <br /> prevent structural weakening as a result of contact with any hazardous substances released from the <br /> primary containment. Immediately contact a properly licensed, trained, and certified contractor to make <br /> repairs to the UST system under permit and inspection of the EHD. The technician removed all liquid <br /> and put it in the facility's hazardous waste drum. <br /> 406. The Diesel STP double wall Sump sensor and the Diesel fill sump liquid sensor failed when tested <br /> for functionality. The sensors monitoring the liquid level in the interstitial space shall be maintained so <br /> that a breach in the primary or secondary containment is detected before the hazardous substance is <br /> released into the environment. Correct immediately by having a properly licensed, trained, and certified <br /> contractor replace the failed component with a functional component (LG 113-x listed, if applicable) and <br /> obtain a permit within one business day from the EHD. The failed sensor was replace, retested and <br /> passed (Model 794390-304). Permit must be obtained on the next business day. <br /> Notes <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, to the EHD within 30 days of <br /> receiving this report. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Muniappa Naidu (209) 468-3439 <br /> EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
