The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Individuals installing UST Systems or components shall possess or work under the direct and <br /> personal supervision of an individual physically present at the work site who possesses all of <br /> the following: — <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> b. A valid, current International Code Council (ICC) UST Installer/Retrofitting certificate <br /> and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed. <br /> 3. Qualified service technicians shall possess or work under the direct and personal supervision <br /> of an individual physically present at the work site who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested. <br /> 4. Each individual installing or testing any component shall hold a certificate of training from the <br /> manufacturer of that component. <br /> 5. All UST components including spill buckets piping, fittings, sumps, and under-dispenser <br /> containment must be certified by an independent testing organization and labeled as required <br /> by the organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of Regulations, <br /> Title 23, Chapter 16 Table B for complete list.). Pursuant to the Underwriters Laboratories Inc <br /> (UL) listings, only those products bearing the UL mark will be considered listed (only <br /> applicable to UL listed products). <br /> 6. All monitoring systems shall be listed on the LG113 (current version). <br /> 7. All installation and testing shall be in compliance with manufacturer's instructions, section <br /> 2635 and section 2636 of CCR and Chapter 6.7 of the Health and Safety Code as amended <br /> and effective July 1, 2003. <br />
