It could not be determined how long the UW had been on <br />site. <br />Immediately have this UW removed and provide this office with a shipping record. <br />Note: Waste alkaline batteries were being stored in boxes with a label that read "Please do NOT deposit alkaline, lithium, or <br />non -rechargeable batteries", from Rechargeable Battery Recycling Corporation <br />Medical Waste Management Act (MWMA) <br />Violation #999 Waste pharmaceuticals are not being transported pursuant to 118000 HSC or treated pursuant <br />to 118215 HSC prior to disposal. Waste pharmaceuticals (pharmaceuticals returned by customers or pharmaceuticals <br />that became waste as a result of contamination of some kind) are disposed of through the Distribution Center (DC) in <br />Woodland, CA. Medical waste haulers are required to be registered hazardous waste haulers through the Department of <br />Toxic Substances Control and approved to haul medical waste through the California Department of Public Health. Waste <br />pharmaceuticals are required to be incinerated [or treated according to 118215(a)] pursuant to the MWMA. This is a violation <br />of 118222(b) HSC. <br />PLEASE NOTE THIS IS NOT A HAZARDOUS WASTE VIOLATION IT IS A CALIFORNIA MEDICAL WASTE <br />MANAGEMENT ACT VIOLATION. <br />Within 30 days, submit a plan for handling, storing, and disposing of waste pharmaceuticals pursuant to the California <br />Medical Waste Management Act. <br />Violation #999 Sharps containers ready for disposal are not being stored pursuant to 118285 HSC. At time of <br />inspection, sharps container ready for disposal was not dated. It could not be determined how long the sharps container had <br />been on site but the Store Manage thought it may have been about one year. <br />PLEASE NOTE THIS IS NOT A HAZARDOUS WASTE VIOLATION IT IS A CALIFORNIA MEDICAL WASTE <br />MANAGEMENT ACT VIOLATION. <br />Immediately begin managing and handling sharps containers pursuant to 118285 HSC. <br />Within 30 days, April 3, 2011 complete and submit a copy of the Return to Compliance Certification, found on the back of <br />page 1 of the checklist, with a statement addressing how each of the above listed violations have been corrected.
