A site plan must be submitted identifying the locations <br /> where monitoring will be performed, all monitoring locations(included but not limited to smart sensors and liquid <br /> sensors) were not identified on the submitted map. Immediately log into the California Environmental Reporting <br /> System(CERS) at http://cers.calepa.ca.gov/and upload a copy of the complete and accurate UST Monitoring Site <br /> Plan. <br /> This is a Class II violation. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 8-19-2016 and an inspection report was issued on 8-20-2016 identifying information <br /> to be submitted to bring this site into compliance. This information was required to be submitted 30 days from receipt <br /> of the inspection report issued on 8-20-210. This information has not been received until 10-27-2016. An operator <br /> that receives an inspection report shall have 30 days to submit a written response that includes a statement <br /> documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written <br /> response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection <br /> report. <br /> This is a Class II violation. <br /> 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of accurate monitoring plans were not available for inspection. The monitoring plan(s) uploaded <br /> into CERS is not current and/or not approved by the EHD. The shut down information section for the UDC sensors <br /> for all tanks indicated "No' for the "UDC Leak Alarm Triggers Automatic Pump Shutdown" question. A copy of these <br /> plans shall be accessible on site at all times. Immediately locate and retain a current and accurate plan copy on <br /> site. <br /> This is a Class II violation. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the <br /> owner or operator of the UST system and all the operating conditions have been met. New Bravo Snap and Lock <br /> Sump Covers were installed at this site, a permit SR0073494 was obtained for this repair. This permit was approved <br /> for the installation of Bravo retrofit covers on fill sumps only, however, during the site inspection it was discovered that <br /> new Bravo Snap and Lock Sump Covers were installed on turbine as well as fill sumps. Emergency repairs relating <br /> to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified <br /> contractor pursuant to the EHD'S policy and a permit obtained within one business day from the EHD. All other <br /> repairs must have a permit submitted to and approved by the EHD prior to beginning work. To date, no additional <br /> documentation was submitted to amend the existing permit SR0073494 with the correct scope of work. Immediately <br /> submit all appropriate documentation, as requested in the 7-14-2016 EHD permit inspection report. <br /> This is a Class II violation. <br /> Page 4 of 7 <br />
