A checklist was left onsite at the time of inspection, and a complete report was delivered to the <br /> facility on August 21, 2013. A cold start inspection for the SR0067435 was also conducted on August 15, <br /> 2013 (post software upgrade on the Veeder Root system). All system functions were verified, a copy of <br /> the alarm history was submitted to the EHD. <br /> Class II Violations: (303, 313, 404, and 406): <br /> 303. The 87-octane STP sump sensor was located approximately 2-inches up the saddle of the tank <br /> sump, the 87 fill sump sensor was located approximately 2-feet up the saddle of the tank sump, and the <br /> diesel fill sump sensor was located approximately 1-foot up the saddle of the tank sump. These sensors <br /> were not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained <br /> to be able to detect a leak at the earliest possible opportunity. The technician repositioned all the <br /> sensors to the lowest points of the respective sumps. CORRECTED. <br /> 313. The 91-octane spill container failed when tested. All spill containers shall have a minimum capacity <br /> of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. The <br /> technician replaced the outer O-ring of the spill container, retested and it passed. A retrofit verification <br /> with the inspector already onsite was filled out and a copy provided to the store manager at the time of <br /> the inspection. CORRECTED. <br /> 404. Water was found in the 87-octane fill sump (approximately 4 gallons), diesel fill sump <br /> (approximately 1 gallon), 87-octane STP (approximately 2 cups), 91 STP sump (approximately 1.5 <br /> quarts), and in the vent sump (approximately 0.5 cup). Technician removed the liquid from above <br /> sumps and retained for disposal. According to the reviewed site records, approximately 100-gallons was <br /> removed from the onsite sumps on 12-10-12, approximately 150-gallons were removed from the 87-fill <br /> sump on 12-26-12, and approximately 230-gallons were removed from the 87-fill sump on 01-08-13. <br /> Secondary containment shall be constructed to prevent any water intrusion into the system by <br /> precipitation, infiltration, or surface runoff.
