Water was found in�all the sump. Secondary containment shall be constructed to prevent any <br /> water intrusion into the system by precipitation, infiltration, or surface runoff. Immediately remove this <br /> liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage <br /> accordingly. Immediately contact a properly licensed, trained, and certified contractor to address the <br /> water intrusion into all the tank sump under permit and inspection of the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility within 30 days of receiving this report. <br /> 106. The monitoring plan on site are not current and not approved by the EHD. The monitoring plan <br /> must be current and approved by the EHD. Complete and submit a copy of the monitoring plan for <br /> approval by the EHD within 30 days of receiving this report. An approved copy will be returned for your <br /> records. A copy was corrected, approved provided for the EHD. <br /> 206. The designated operator employee training for Reshbea Holsome (Hired on 9/20/12) was not <br /> performed. The designated operator shall train facility employees for which he or she is responsible in <br /> the proper operation and maintenance of the UST system once every 12 months. The training shall <br /> include, but is not limited to, <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
