The Diesel 87 and 91 spill container failed when tested. All spill containers shall have a minimum <br /> capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br /> Immediately contact a properly licensed, trained, and certified contractor to repair or replace the spill <br /> container under permit and inspection of the EHD. The drain valve was cleaned and the o-ring was <br /> checked on the Phil-tite spill bucket, retested and passed. <br /> 314. The light used as part of the overfill prevention system is not currently functional on the UST <br /> system. The Light bulb was replaced and retested during the inspection. <br /> Notes <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation to the EHD within 30 days of receiving <br /> this report. <br /> Water intrusion in the tank sumps is a repeat violation and needs to be investigated immediately. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY ITIME AT EHD'S CURRENT H URLY RATE. <br /> EHD Inspector: Recei By: Title:li <br /> ^v tA--- 11 1 '/K"( <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
