Monitoring System Alarm Logs maintained by facility's employees were not found on site. There <br /> were alarms every single month but no documentation can be located. These records shall be kept on <br /> site for at least 3 years. Immediately locate the Log, document any alarms from the Veeder-Root <br /> monitoring system (date, alarm type, and response action if taken) and maintain the Log on site. <br /> 303. The 87 STP sump senor and vent annular sump sensor were not working properly to ensure <br /> continuous monitoring of the tank system as required by CCR 2630(d). Both sensors were replaced, <br /> retested during the inspection and inspector witnessed sensors passing testing. <br /> 402. A diesel/ brine leak was found at the 91/diesel split tank on October 29, 2013 by Xerxes Quality <br /> Control Manager. All primary containment must be impervious to the liquid and vapor of the substance <br /> contained. Immediately cease any deposition of diesel and have a properly licensed, trained, and <br /> certified contractor replace or repair the failed diesel compartment under permit and inspection of the <br /> EHD. Per Xerxes Quality Control Manager, the failed diesel compartment interstitial can't be repaired, <br /> the diesel UST system must be placed out of operations. <br /> 403. Annular 91/diesel brine was found in the diesel tank indicating a leak in the secondary <br /> containment. The confirmation of brine weeping into the diesel tank was confirmed on October 29, 2013 <br /> by Xerxes Quality Control Manager. Secondary containment shall be impervious to the liquid and vapor <br /> of the substance contained and constructed to prevent structural weakening as a result of contact with <br /> any hazardous substances released from the primary containment. Per Xerxes Quality Control <br /> Manager, the failed diesel compartment interstitial can't be repaired, the diesel UST system must be <br /> placed out of operations. In addition, because the secondary containment of the 91/diesel UST is not <br /> continuously maintained, the 91 UST system must be placed out of operations. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPEC N AIJANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> InspectorReceiv By Title: <br /> EHD 23-02 Rev 04/19/12 Pa 3 UST INSPECTION REPORT <br />
