7.1.2. The Xerxes site supervisor should conduct a prejob planning meeting with everyone <br /> involved in tank-entry work to review site-specific elements that may impact safety. <br /> 7.2. Site-Specific Safety Information <br /> 7.2.1. Complete the Site-Specific Safety Plan Checklist (Appendix B). <br /> 7.2.2. The Xerxes site supervisor must obtain available site-specific information from the <br /> customer/contractor on permit-required confined space hazards (including emergency- <br /> response procedures and any potential fire, explosion, health, safety or other hazards) and <br /> share this information with other crew members before tank-entry work. The <br /> customer/contractor must: <br /> 7.2.2.1. provide you with information on the elements, (including identified hazards and <br /> their first-hand knowledge of the space if any), that make the space a permit-required <br /> confined space; <br /> 7.2.2.2. provide you with information on any precautions or procedures they have <br /> performed in or near the permit-required confined spaces; <br /> 7.2.2.3. coordinate entry operations with you before you and their employees will be <br /> working in or near permit-required confined spaces; <br /> 7.2.2.4. debrief you at the conclusion of entry operations regarding the permit-required <br /> confined space program followed and any hazards confronted or created during entry <br /> operations. <br /> 7.3. Personal Protective Equipment (PPE) <br /> 7.3.1. In addition to wearing basic construction-site PPE, such as a hard hat, safety <br /> glasses with side shields, steel-toed shoes, and safety vest, wear protective coveralls <br /> while inside a tank. Wear protective gloves to prevent skin contact with chemicals where <br /> necessary. If you will be grinding, wear an approved dust mask and goggles or face shield. <br /> 7.4. Tank Entry Work Permit ("Permit") <br /> 7.4.1. General <br /> 7.4.1.1. A written Tank Entry Work Permit (Appendix C) must be completed by the site <br /> supervisor before authorized tank entrants may enter a tank (break the plane of the <br /> manway). <br /> 7.4.2. Site Supervisor Responsibilities <br /> 7.4.2.1. Complete a permit if tank entry is required. By completing and issuing a permit, <br /> you are verifying that all the steps do the permit have been addressed prior to tank entry <br /> 21 <br />
