❑ If the tank has held fuel, or you have reason to believe that flammable liquids may be present in
the work area, sample the work area with your combustible-gas indicator at several locations
and record the results below. If you get a reading above 0% LEL, determine where the vapors
are coming from before attempting to perform work. Do not perform work if the indicator reads
above 10%LEL. Refer to Table 1 for more information on this subject
Time of
Day:
Oxygen
(%)
Flammable
Gas(iaLEL)
Toxic Gas
(PPM)
Tester's
Initials
Physical Hazards — Evaluate the site for any physical hazards that you may encounter:
❑ Verify electric cords and lights are in good shape with no defects or exposed wires.
❑ If you will be working in very warm temperatures, plan your schedule so that you allow frequent,
short rest breaks (out of direct sunlight if possible). Drink plenty of water.
❑ Be aware that heavy equipment operators may not always be able to see you. Plan your work
so that you stay out of the path of this moving equipment.
❑ If the tank you will be working on Is buried, determine if the excavation has been properly sloped
or shored. Refer to Appendix H, "Excavation Safety,"which contains detailed safety information
for working around excavations. If the excavation does not look correct, immediately contact the
site supervisor or the plant manager for assistance.
❑ Falls are one of the most common causes of injury at a jobsite. Refer to Appendix I, "Working at
Elevated Heights—Fall Protection Supplement,"which contains detailed safety information on
this subject. Keep your work areas organized and pick up debris that could become tripping
hazards.
Environmental Hazards —Weather can play a major role in field service work.
❑ Cease work activities at the approach of any electrical(lightning)storm.
❑ Determine what your plan will be if you are faced with snow, Ice, rain or extreme changes in
temperature.
❑ Cleanup all debris generated at the site. Bring any waste that may-he considered hazardous
back to the plant for proper disposal.
I
