XERXES <br /> TANK ENTRY WORK PERMIT <br /> Customer: SA# Date: <br /> r <br /> Site Location: Tank to be Entered: <br /> This Tank Entry Work Permit must be completed in its entirety by the site supervisor before beginning tank- <br /> entry work. The duration of this permit may not exceed the time required to complete the assigned job. After <br /> all items have been verified by the site supervisor, the permit shall be posted at the tank-entry location before <br /> and during all tank-entry work. <br /> Purpose of Tank Entry: Duration: Start: Finish: <br /> Name and Title of all Parties on Site: <br /> Location of Telephone to Use in Emergency: <br /> Local Emergency Medical Phone # Fire # Police # <br /> Route for Emergency Evacuation: <br /> Part A— Before Tank Entry (Site supervisor must initial each item when completed.) <br /> 1.All crew members are wearing required personal protective equipment. <br /> 2.Work area protection is set up properly. <br /> 3. Excavation Is sloped or shored properly. <br /> 4. If tank entry is through a wall, the work area Is exposed to a minimum of 6 feet x 6 feet. <br /> 5. All potential hazardous energy is controlled by disconnecting and by locking out and tagging out. <br /> 6.Tank is emptied,cleaned and pumped of product. <br /> 7. All available plugs are removed for defuming. <br /> 8. Tank is isolated; all external sources of product are capped or disconnected. <br /> 9. Pump is removed and manifolded vents are disconnected. <br /> 10.All potential ignition sources are identified and removed. <br /> 11. Venturi with 12-foot extension is installed and grounded,and is exhausting air from tank. <br /> 12. Attendant and crew members have been instructed on their safety duties. <br /> 13. Gas/oxygen instrument has been calibrated and function-tested. <br /> 14. Safety harness, tripod and retrieval line are inspected and set up. <br /> 15. Only air tools (not electrical) are being used. <br /> 16. Communication procedures between authorized tank entrant and attendant have been <br /> established. <br /> 17. Fire extinguishers, air horn and eyewash bottles have been checked, and are readily available. <br /> 18. Integrity and condition of the tank have been reviewed, and the-':ank is safe to enter. <br /> 19. No smoking signs and Tank Entry Work Permit are posted adjacent to manway. <br /> FormTEWP1 Elf.2/06 <br />
