Hazelton Avenue
Stockton, California 95205
Subject: Application for Encroachment Permit
Moore Petroleum Bulk Plant
5491 F Street, Banta, California
Apex Project#ERA02.004
Dear Mr. John Wotila:
Apex Envirotech, Inc. (Apex) has been authorized by our client to submit.an application
for a.permit-to encroach on public property at the above referenced site (Figure 1 and 2).
As per Apex's conversation with the San Joaquin County Public Works Division
(SJCPWD) on May 25, 2004, please find the enclosed material: an encroachment permit
application form(Appendix A),a copy of liability insurance with San Joaquin County as
additionally insured(Appendix B), a$2,000 performance bond per well in county right-
of-way(Appendix C), ozone sparge well construction diagrams, and a Cal Trans
designed traffic plan.
'Drilling is tentatively scheduled for October, 2004 and will last two days. This consists
of four 8-inch borings along the public right of way on the east side of Sixth Street
(Figure 3). Also, at a later date, two trenches 87in. wide and 18 in. deep will be cut. One
trench will be in the dirt 150 feet in length paralleling Sixth Street, and the other cutting
the asphalt of Sixth Street to cross its width, in order to connect ozone supply lines to the
wells (Figure 4). Trenching will also be a two-day project. Each occurrence will involve
a temporary lane closure along the effected section of roadway.
Traffic Plan:
The proposed traffic plan is based on Chapter 5 of the California Department of
Transportation's standard Traffic Operations manual, in concern of public safety and
traffic flow (Figure 5). For all proposed activities, arrangement of traffic channeling
equipment is dependent on a varying work area length. The lane closure length will be
kept minimal, allowing 25 feet of buffer space between actual work area and the end of
the lane taper.
