Box <br /> 2048 Stockton, CA 95201, (209) 948-3656. All work under this permit must meet with the <br /> approval of the State Representative. <br /> Failure on the Permittee's part to comply with any provision will be cause for revocation of this <br /> permit. <br /> IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, <br /> PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY <br /> GRANTOR. <br /> Pemmittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State <br /> Standard Specifications Reference to Engineer in the State Standard Specifications shall include <br /> State Representative. <br /> Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, and <br /> Business and Professions Code, Section 8771. Permittee shall physically inspect the work site <br /> and locate survey monuments prior to work commencement. Monuments shall be referenced or <br /> reset in accordance with Business and Professions Code. <br /> A pre-job conference with the Permittee, Contractor, and State Representative may be required <br /> prior to start of work. Permittee shall contact the State Representative and arrange the meeting. <br /> This meeting may be waived at the discretion of the State Representative. <br /> Notwithstanding General Provision #4, your contractor is required to apply for and obtain an <br /> encroachment permit prior to start of work. <br /> Permittee shall, prior to commencement of any work, provide the State Representative with the <br /> name and phone number of the person in responsible charge of the work to be performed <br /> under this permit. <br /> In accordance with Section 6500 of the Labor Code, a Cal-OSHA permit is required prior to the <br /> start of work. <br /> This permit is not valid until Permittee has obtained permission from adjacent property owners, <br /> Irrigation Districts, Cities, Counties or other interested parties to perform the proposed work. <br /> In accepting this permit, the Permittee agrees that when notified by the State to relocate his <br /> facilities to accommodate proposed freeway construction, that said facilities will be relocated by <br /> the Permittee at Permittee's expense and Permittee hereby waives any claim for reimbursement <br /> for the required relocation under Section 703 of the Streets and Highways Code. <br /> Except when necessary, as determined by the State Representative, no installation, construction <br /> equipment or personal vehicles shall operate or park within the traveled way. <br /> Welded steel pipe shall be jacked or bored, without the use of water as a jet, under the pavement <br /> and improved surfacing, without disturbing same.
