Box 2048 Stockton, CA 95201, (209) 948-3656. All work under this permit must meet with the approval of the State Representative. Failure on the Permittee's part to comply with any provision will be cause for revocation of this permit. IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY GRANTOR. Pemmittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State Standard Specifications Reference to Engineer in the State Standard Specifications shall include State Representative. Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, and Business and Professions Code, Section 8771. Permittee shall physically inspect the work site and locate survey monuments prior to work commencement. Monuments shall be referenced or reset in accordance with Business and Professions Code. A pre-job conference with the Permittee, Contractor, and State Representative may be required prior to start of work. Permittee shall contact the State Representative and arrange the meeting. This meeting may be waived at the discretion of the State Representative. Notwithstanding General Provision #4, your contractor is required to apply for and obtain an encroachment permit prior to start of work. Permittee shall, prior to commencement of any work, provide the State Representative with the name and phone number of the person in responsible charge of the work to be performed under this permit. In accordance with Section 6500 of the Labor Code, a Cal-OSHA permit is required prior to the start of work. This permit is not valid until Permittee has obtained permission from adjacent property owners, Irrigation Districts, Cities, Counties or other interested parties to perform the proposed work. In accepting this permit, the Permittee agrees that when notified by the State to relocate his facilities to accommodate proposed freeway construction, that said facilities will be relocated by the Permittee at Permittee's expense and Permittee hereby waives any claim for reimbursement for the required relocation under Section 703 of the Streets and Highways Code. Except when necessary, as determined by the State Representative, no installation, construction equipment or personal vehicles shall operate or park within the traveled way. Welded steel pipe shall be jacked or bored, without the use of water as a jet, under the pavement and improved surfacing, without disturbing same.
