The work outlined in <br /> this Work Plan was to have been completed within 90 days of the PHS/EHD approval <br /> date. The work has not been completed. This site is not in compliance with PHS/EHD <br /> directives. The work as outlined in the approved Work Plan must be completed by <br /> August 25 , 2000. <br /> A well permit application and $ 89.00 permit fee should be submitted to PHS/EHD as <br /> soon as possible. After the permit has been issued the work can be scheduled.
