There is some discrepancy in your application. The <br /> "Contaminated Site Description" on page 4 indicates there were two 5,000-gallon USTs removed in <br /> September 1989. The "Preliminary Investigation and Evaluation Report" dated September 15, 1997 <br /> states : "In September 1989, one 1 ,000-gallon underground storage tank (UST), dispenser and piping <br /> were removed from the site." <br /> To determine eligibility for Priority Class B, the Fund looks to the gross annual receipts for the three <br /> years preceding the application. You submitted the federal tax returns 1120(s) for Guardino- <br /> Crawford Co ., Inc. Also required are the federal tax returns 1040(s) for the same three years for the <br /> owner or majority shareholders of the corporation and any other federal tax returns as may apply <br /> according to the enclosed chart.
