Underground Storage Tanks —Tank Forms — Provide all current information in <br /> CERS. Complete a separate tank form for each tank. (The form is appended.) <br /> 4. Certification of Financial Responsibility/Letter from Chief Financial <br /> Officer: Complete the appended forms, and upload in CERS. This document is <br /> required to be submitted annually. <br /> 5. Underground Storage Tank Monitoring and Response Plans: Upload in CERS. <br /> Retain an approved copy of these plans on site for future reference. (The forms <br /> are appended.) <br /> 6. If you are the owner, you must submit a notification form identifying the <br /> "Designated UST Operator" for each facility owned. <br /> 7. Report#5021: Update all information for all programs at this site including the <br /> UST and Hazardous Waste programs, sign and date. Bring with all other forms to <br /> our accounting office to pay for transfer fees. (A form is appended.) <br /> 8. Go to: http.//www.waterboards.ca.gov/cwphome/ust/regulatory/index.html#rqs to <br /> obtain the current statutes (Underground Storage of Hazardous Substances) and <br /> 5 <br />
