Appended for your information is a brochure of Best Management Practices <br /> (BMPs) for Retail Gasoline Outlets that you can use as part of your BMPs for <br /> your facility. <br /> 10.Hazardous Materials Management Plan: Contact EHD Hazmat Staff at (209) <br /> 468-3420 for information on updating this plan on CERS. <br /> 11.Information and instructions for obtaining a permanent California EPA ID number <br /> (The form is appended.): Complete the application for a California Hazardous <br /> Waste Permanent ID Number and submit it to the Department of Toxic <br /> Substances Control (DTSC) by fax, mail, or email. <br /> 12.Board of Equalization UST Storage Fee Account Number— Call (916) 322-9669 <br /> to obtain a current number for your facility. <br /> 13.San Joaquin Valley Air Pollution Control District — Call (209) 557-6400 to obtain <br /> an operating permit for the UST vapor recovery system. <br /> 14.Food facility operations — Call Vidal Pedraza at (209) 468-0334 for operating <br /> permit information for opening the retail food facility. Please consider attending one of these free hazardous waste or <br /> underground storage tank classes. <br /> The following is the required follow up and routine testing for your facility: <br /> 1. Annual monitoring system certification, leak detector testing, and spill container <br /> testing were last performed on August 19, 2016. These tests are required once <br /> every 12 months. <br /> 2. Secondary containment testing was conducted on August 17 2016. Secondary <br /> containment testing is required once every 36 months.
