Any material that meets or <br /> exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br /> inventory, and submit to the EHD for approval. <br /> The Hazardous Materials Inventory forms for the bleach, acetylene, and oxygen were not complete and accurate. <br /> The maximum amount of bleach is 900 gallons vs. 300 gallons; the maximum amount of acetylene is 505 cubic feet <br /> vs. 50 pounds, and the oxygen is reported as 1,000 pounds vs. 403 cubic feet. This information must be current <br /> and submitted as part of the business plan. Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The maps did not show/address: <br /> 1. Loading areas. <br /> 2. Internal roads. <br /> 3. Adjacent streets. <br /> 4. Storm and sewer drains. <br /> 5. Access and exit points. <br /> 6. Emergency response equipment (fire extinguishers and absorbent). <br /> 7. Emergency shutoffs. <br /> 8. Evacuation staging area. <br /> 9. The map does not show the hazardous material handling and storage areas for the two 55 gallon waste oil <br /> drum, two 300 gallon totes of sodium hydroxide, twelve 8 gallon propane cylinders, and sixty 25 pound containers of <br /> Freon. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as "NA".
