A site-specific Health and Safety Plan (HASP) will be prepared prior to the investigation activities. The <br /> HASP will be prepared in general accordance with the Occupational Safety and Health Act (OSHA). A <br /> health and safety meeting with the utility locator and drilling subcontractors will be conducted on site prior to <br /> start of the investigation activities. A copy of the HASP will be maintained with Ash Creek personnel at the <br /> site throughout the investigation activities. <br /> Ash Creek will obtain the required well permits from San Joaquin County Environmental Heath Department <br /> (SJEHD) prior to field activities. During field work, Ash Creek will coordinate the required inspections with <br /> the SJEHD. Ash Creek will also obtain approval and/or necessary permits from the Port of Stockton and <br /> Union Pacific (UP) for completing the work. If necessary, Ash Creek will obtain flaggers to work near the <br /> railroad tracks. <br /> Monitoring Well Abandonment. Field documentation will be conducted during field activities consistent <br /> with Ash Creek Standard Operating Procedure(SOP) 1.1, included in Appendix A. <br /> Monitoring wells PS/WC-2S and PS/WC-2M will be abandoned to accommodate the proposed railroad spur. <br /> PS/WC-2M is constructed to a total depth of 34 feet bgs and includes a screened interval from 29 to 34 feet <br /> bgs. PSM/C-2S is constructed to a total depth of 19 feet bgs and includes a screened interval from 5 to <br /> 19 feet bgs. Both are completed with a flush mount well monument. Construction details are included in <br /> Appendix B. Due to the presence of dissolved-phase hydrocarbon concentrations in groundwater from <br /> PS/WC-2S, this well will be perforated by over-drilling before grouting from the bottom to the surface. <br /> Monitoring well PS-WC-2M will be pressure grouted in place up to a depth of 2.5 feet below grade, cut at a <br /> depth of 2.5 feet below grade(both conductor and well casings), capped with a Type 1/II concrete mushroom <br /> cap, and backfilled with clean fill. SJEHD will be notified prior to field work and appropriate permits will be <br /> obtained. <br /> Monitoring Well Installation. Ash Creek will oversee the installation of two replacement monitoring wells <br /> (PS/WC-2S-R and PS/WC-2M-R; Figure 2) installed using hollow-stem auger drilling techniques. The two <br /> wells will be re-located approximately 50 feet to the east of their current locations. The proposed locations <br /> are outside of the proposed construction zone, and will provide data equivalent to data obtained from <br />■' original well locations. Prior to well installation, each location will be cleared by hand auger to a depth of <br /> 5 feet bgs per NuStar protocols. Soil sampling will occur continuously and soil will be logged in general <br /> I' accordance with ASTM 2487/2488. Soil logging will be conducted to confirm the proposed screened <br /> intervals and field screening will be conducted consistent with SOP 2.1, contained in Appendix A. Soil <br /> samples are not expected to be collected for chemical analysis due to existing chemical data at the <br /> locations from previous investigations.
