Manifest numbers 001810118JJK was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DISC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. This is a repeat violation, Class II. 112 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. Only one 2013 consolidated manifest for 160 gallons was available during this inspection; copies of all hazardous waste disposal records for 2013 were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. This is a repeat violation, Class IL 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. One 55-gallon drum of used oil was not closed in the hazardous waste storage area. All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. This is a repeat violation, Class 11. 601 CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. The following hazardous waste were stored longer than 180 days, or considered to be stored longer than 180 days due to lack of accumulation start date on the label: —Three 55-gallon hazardous waste drums with a start date of 3/18/14, 6/1/14, (red drum)4/4/14 at the hazardous waste storage area; — Four 5-gallon hazardous waste buckets (used grease, metal shavings)with no start date, three 5-gallon hazardous waste buckets with start date of 2/13/14, 3/18/14, and 5/23/14, and one 5-gallon bucket of used oil with no hazardous waste label, all located at the hazardous waste storage area. — Four 55-gallon drums of used oil by storage shelves had no label. —Two 5-gallon bucket under the work bench, one with no label, the other with no start date on the label. Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. This is a repeat violation, Class 11.
