THIS VERSION SUPERSEDES ALL PREVIOUS VERSIONS. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 1/7/2015. <br /> Consent to perform the inspection and collect samples was given by Nina Wong-Hamamoto, Safety Health <br /> Environmental Supervisor. Permission to photo was not granted per facility policy. <br /> This facility generated 3.7345 tons of waste in the last calendar year(Jan-Dec). Facility will be re-classified to <5 <br /> tons generator category for2015. Nina stated the drop in waste generated is because they are now operating at <br /> a lower capacity with new owner. <br /> Business owner of the facility has been changed to R & B Foods, Inc., and the new EPA ID# is CAL000400587; <br /> previous EPA ID# CAD115092025 has been inactivated on 9/17/2014. <br /> 2 boxes of high wattage lamps were observed outside the Universal Waste Lamps storage area during this <br /> inspection. Nina stated they are not waste at the moment as they are looking for potential buyers, but may <br /> dispose of them if they couldn't find a buyer. Please be aware if these lamps are declared waste, you are <br /> required to make a hazardous waste determination and dispose of them accordingly. If they are determined to be <br /> universal waste (i.e. sodium lamps, etc.), disposal must be made in accordance to Universal Waste Rules under <br /> Title 22. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information <br /> online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
