Each phase of this project will be completed with a separate permit <br /> application (attached). This work was proposed in Stantec's April 24, 2009 Remedial Excavation <br /> Work Plan and was approved by the California Regional Water Quality Control Board (RWQCB) in <br /> a letter dated May 22, 2009 (Attached). <br /> The attached permit applications include the following; <br /> • Well permit application for the advancement of six soil borings; <br /> o Master File Record (MFR) <br /> o Licensed Contractor Declaration (LCD) <br /> o Permit fees ($404.00) <br /> o Site Plan <br /> • Well permit application for the destruction of three monitoring Wells (MW1/B2, MW-2, and <br /> MW-3) <br /> o LCD <br /> o Permit fees ($60.00) <br /> o Site Plan <br /> • Well destruction permit for the destruction of a'domestic well <br /> o Permit fees($150.00) <br /> o Site Plan <br /> 6 <br /> Drilling activities are tentatively scheduled to begin 2009. The first phase of the project <br /> will include the advancement of up to six soil borings as described in the attached work plan. <br /> Before any excavation work will take place Underground Surface Alert will be notified, a private <br /> utility locate company will be contracted, and the proposed work locations will be cleared down to <br /> five feet below ground surface (bgs) for utilities. <br /> I:\PG&E\French Camp\2009 remediation\Permits\Permit App Cvr Ur 6-9-09.doc <br /> M <br />
