Each phase of this project will be completed with a separate permit <br /> application (attached). This work was proposed in Stantec's April 24, 2009 Remedial Excavation <br /> Work Plan and was approved by the California Regional Water Quality Control Board (RWQCB) in <br /> a letter dated May 22, 2009 (Attached). <br /> The attached permit applications include the following; <br /> • Well permit application for the advancement of six soil borings; <br /> o Master File Record (MFR) <br /> o Licensed Contractor Declaration (LCD) <br /> o Permit fees ($404.00) <br /> o Site Plan <br /> • Well permit application for the destruction of three monitoring Wells (MW1/B2, MW-2, and <br /> MW-3) <br /> o LCD <br /> o Permit fees ($60.00) <br /> o Site Plan <br /> • Well destruction permit for the destruction of a'domestic well <br /> o Permit fees($150.00) <br /> o Site Plan <br /> 6 <br /> Drilling activities are tentatively scheduled to begin 2009.
