Box 942732 <br /> Sacramento, CA 94234-7320 <br /> Attention: Tej Pahwa and John Hart <br /> A brief final report of the results of the thermal treatment process and <br /> the information obtained will be -&ubmitted to DHS within one month of <br /> the conclusion of the site remediation activities. The report shall be <br /> submitted as a draft report, subject to DHS review, before finalizing. <br /> 4. The contacts for DHS will be Tej Pahwa and John Hart at (916) 324-1807. <br /> DHS reserves the right to change the contacts for DHS by either <br /> telephone or written notification. <br /> 5. Inspection and Entry. Ogden shall allow the Department or persons <br /> authorized by the Department, upon the presentation of credentials and <br /> other documents as may be required by law, to: <br /> a. Enter at all times upon the site's premises; <br /> b. Have access to and copy, at all times, any records that must be kept <br /> under the conditions of this variance; <br /> C. Inspect at all times any facilities, equipment (including monitoring <br /> and control equipment) ,- practices, or operations regulated or <br /> 'required under this variance; and <br /> d. Sample or monitor, at all times for the purposes of assuring <br /> variance compliance, any substances or parameters at any location. <br /> 6. The DHS contact shall be notified at least five (5) working days before <br /> start-up of the demonstration. <br /> 7. Arrangements With Local Authorities. Ogden shall maintain arrangements <br /> with State and local authorities as required by 40 C.F.R. , Section <br /> 264.37. <br /> 8. Protection of Human Health and the Environment. DHS may order an <br /> immediate termination of all operations under this variance at any time <br /> it is determined that termination is necessary to protect human health <br /> and the environment or at any time it is determined that operations are <br /> not in compliance with the conditions of this variance. <br /> 9. Ogden shall notify DHS within 24 hours by telephone and within 72 hours <br /> in writing if any emergencies or hazardous waste releases occur during <br /> the operation of this facility. <br />
