Remedial Action Work Plan e Response Page 2 of 2 <br /> Barnes Trucking April 23, 2015 <br /> 1817 Fresno Avenue <br /> Stockton, California 95206 <br /> Mix neat cement at a ratio of 5 to 6 gallons of clean, potable water per 94 pounds of <br /> portland cement; <br /> As Stratus proposes to use above-ground tubing to connect the OI system to the ozone <br /> injection wells , the EHD requires that all appropriate actions be taken to ensure that the <br /> tubing is protected from damage and well heads are water-tight; and <br /> o All investigation-derived wastes, including soil cuttings, debris, and groundwater <br /> generated from soil borings and well installations , shall be containerized for proper <br /> disposal and the containers labeled to characterize the waste: hazards, composition, <br /> generation site address, generator's name, and date of accumulation. <br /> Any remediation plan that entails subsurface injection of oxidizing chemicals requires the <br /> approval of the CVRWQCB before such technology can receive the final approval of this agency <br /> for this work plan implementation . Additional inspection time spent beyond the initial pre-paid fee will be at the <br /> current hourly rate of $ 130. <br /> The EHD will contact your consultant when the permit is issued . After permit issuance , the EHD <br /> requires well inspections to be scheduled with Johnny Yoakum , REHS , at least 5 workdays prior <br /> to implementing fieldwork.
