The following is an itemized list of underground storage tank violations that have not <br /> been addressed for KWIK SERV LODI BW 113* as of October 07, 2020. <br /> Open violations from January 09, 2020 inspection <br /> Violation#208-Failure to conduct the designated UST operator visual inspection at least once every 30 <br /> days. <br /> 1)OBSERVATION:The designated operator failed to check and note the correct test dates for the Secondary <br /> Containment Testing on the 1/2019-1-2020 designated operator inspection reports. Secondary Containment <br /> Testing was conducted on 1/16/2018. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> 2)OBSERVATION: Designated operator reports were not signed within the required time frame. <br /> -The 1/11/2019 designated operator report was not signed by the owner/operator. <br /> REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, <br /> the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 48 <br /> hours of being provided a signed copy of the report,the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br /> issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#214-Failure to comply with UST operating permit conditions. <br /> OBSERVATION:According to the replacement notifications sent to EHD on 9/10/2019 and 11/1/2019, the 87 <br /> product leak detector was replaced on 9/9/2019 and the 91 product leak detector was replaced on 10/27/2019;work <br /> orders or testing documentation indicating the replacement were not found on site at the time of the inspection. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the replacement of the leak detectors on the <br /> 87 and 91 tanks.
