The following is an itemized list of underground storage tank violations that have not <br /> been addressed for YOSEMITE PETROLEUM as of May 14, 2020. <br /> Open violations from January 29, 2020 inspection <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #202 -Failure to maintain the "Facility Employee Training Certificate" on-site(after October 1, <br /> 2018). <br /> OBSERVATION On the Facility Employee Training Certificate dated 10/18/2019 the "initial training date" listed for <br /> the employee, Zuleika Taveras was after the"date of assuming responsibility as a facility employee". Training for <br /> Amrik and Nirmal was conducted in November but recorded on the 10/18/2019 training certificate. <br /> REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#206 - Failure to comply with UST operating permit conditions. <br /> OBSERVATION According to the replacement notifications sent to EHD on 12/23/2019 and 12/19/2019, VMI 99 <br /> LD 2000 leak detectors were replaced on 12/23/2019 and 12/19/2019 on the 87 tank; work orders or testing <br /> documentation indicating these replacements were not found on site at the time of the inspection. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed, trained, and certified contractor pursuant to the EHD's policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the [UST COMPONENT REPAIR OR <br /> REPLACEMENT] .
