(Geomatrix), <br /> oversaw the destruction of four monitoring wells and eleven extraction wells at the subject site <br /> from 22 September through.25 September 1998. Specifically, the monitoring wells destroyed <br /> were MW-3,MW-9,MW-11, MW-13 and the extraction wells destroyed were EX-1 through <br /> EX-5 and EX-7 through EX-12: The destruction was performed by Gregg Drilling and <br /> Testing, Inc. under permit from your office. Destruction was accomplished by overdrilling the <br /> casing and annular fill materials,pulling the casing from the ground, filling the boreholes to <br /> within 3 feet of the ground.surface with portland cement, and filling the upper 3 feet of the <br /> borehole with-soil. These well-destruction activities were approved in a 15 September 1998 <br /> letter from Kit Custis of the California Regional Water Quality Control Board(RWQCB)to <br /> Ms. Katie L.
