STANDARD OPERATING PROCEDURE <br /> FOR PROPERLY DESTROYING GROUNDWATER MONITORING <br /> AND EXTRACTION WELLS <br /> Corral Hollow and Grant Line Roads Site <br /> ( Tracy, California <br /> Objectives: <br /> • Properly destroy five groundwater monitoring wells (MW-5, MW-6, MW-7, MW-8, and <br /> MW-12) and one groundwater extraction well (EX-6)to eliminate potential conduits to <br /> groundwater. <br /> Site Specific Issues: <br /> • Notify Thomas Ward, the inspector for the City of Tracy, when you arrive onsite. <br /> • Notify Mike Infurna with the San Joaquin County Environmental Health Department <br /> PRIOR to destroying wells. <br /> • Encroachment permit with the City of Tracy required to conduct work in right-of-way. <br /> Standard Procedures: <br /> 1.0 Notifications <br /> 1.1 Contact Mike Infurna with the San Joaquin County Environmental Health <br /> Department PRIOR to the start of the destruction activities. He can be reached <br /> at 209.468.3454. (0) or 209.815.8329 (C). <br /> 1.2 Contact Thomas Ward, inspector for the City of Tracy, to let him know you are <br /> onsite and when you will be leaving. He can be reached at 209.969.1524 (C). <br /> 2.0 Health and Safety <br /> 2.1 Conduct Health and Safety meeting at the start of each work day. Personnel <br /> must read and abide by all the requirements presented in the site-specific Health <br /> and Safety Plan. Review Job Loss Analysis. Complete,Chevron General Permit <br /> to Work. <br /> 3.0 Well Destruction <br /> 3.1 Gauge the well to determine the total depth. Compare the measured depth with <br /> the well construction log. <br /> 3.2 Remove well box and all concrete securing the well box. <br /> 3.3 Using a hollow-stem auger rig, drill down the center of the polyvinyl chloride <br /> (PVC)well casing. The diameter of the auger must be large enough to remove <br /> all material within the original borehole. Use a 10-inch diameter auger to drill out <br /> wells MW-5, MW-6, MW-7, MW-B, and EX-6; and a 12-inch diameter auger to <br /> drill out well MW-12. <br /> 3.4 Advance the auger to the total depth of the well, removing all well casing, filter <br /> pack, and annular seal material. <br /> *2060 SSS <br /> j (tet/ K <br /> �a <br />
