PHYSICAL AND OTHER HAZARDS <br /> (check If <br /> HAZARD hazard is Description <br /> likely to be <br /> encountered <br /> at site) <br /> Drilling and sampling activities may be conducted during cold or <br /> warm weather. Shelter, warm clothing, shade and water will be <br /> Heat/Cold Stress: available at the work site. Work hours will be evaluated based <br /> on actual temperature and weather conditions during the field <br /> activities. See HSP Subsection 4.2.1 for additional hazards and <br /> control measures. <br /> Noise-generating equipment will be used for drilling borings. <br /> Ambient noise levels expected to be representative of a <br /> Excessive Noise: ✓ commercial area (large trucks and equipment may be active on <br /> neighboring properties. See HSP Subsection 4.2.2 for additional <br /> hazards, monitoring and control measures. <br /> Open Excavations: If present at the site, personnel will not enter excavations or <br /> ditches to collect samples or en a e in other activities. <br /> Movement of drill rigs and other heavy equipment at the site and <br /> Heavy Equipment ✓ along adjacent roadways could pose a hazard to workers and the <br /> Operation: public. See HSP Subsection 4.2.5 for additional information and <br /> control measures. <br /> Tailgate safety briefings will be conducted prior to field activities <br /> to identify and avoid potential hazards (unstable structures or <br /> Slip,Trip,Fall surfaces, uneven terrain, steep grades, rocks, holes, or animal <br /> ✓ burrows,); remove or clear, as necessary to reach work area, tall <br /> Hazards: or dense vegetation; use of the "buddy system" to ensure <br /> immediate assistance; adherence to safe work practices; wear <br /> appropriate clothin (steel-toed safety boots). <br /> Skeleto-muscle Lifting or carrying heavy or cumbersome objects without <br /> Injury appropriate assistance or equipment will be prohibited. See HSP <br /> Subsection 4.2.4 for additional information and control measures. <br /> If there is a potential for presence of insects, spiders, snakes, <br /> Biological Hazards: and/or poisonous/irritating plants (poison oak, poison ivy) at the <br /> work site refer to HSP Section 4.2.9 for a discussion of the <br /> hazards and control measures. <br /> SSC and PM to determine whether field activities are to be <br /> Adverse Weather: discontinued due to severe conditions (e.g., high winds,electrical <br /> storms, rain/snow, fog) that could pose a safety hazard. Refer to <br /> HSP subsection 4.2.8 for additional information. <br /> Entry into any confined space is prohibited unless appropriate <br /> Confined Space: measures are implemented in accordance with an approved <br /> Confined Space Entry Permit (see HSP Subsection 4.2.6 for a <br /> discussion of the process,requirements,and control measures. <br /> Underground utilities will have to be surveyed (e.g., USA) and <br /> Overhead or marked prior to any intrusive activities. For potential hazards <br /> Underground r/ associated with utilities(contact with overhead power lines).
