SA N J o A Q U I N Environmental Health Department <br /> C_ 0U '`� 1 Y <br /> Complaint Record Daily Time and Activity Summary from 1/9/2019 to <br /> 1/9/2020 for COAZOOOH6 located at <br /> 17000 E HWY 120 <br /> Activity Date Recorded by <br /> 11/05/2019 At 10 am today I left the office to follow-up on the complaint. I arrived onsite at PE 2546 SAEED <br /> 180 min 10:25am. I waited for Jeff Atkinson until 10:45am. I was told he was on the roof SC 544 <br /> and did not get the text message that I am onsite. I inspected the pump, where the <br /> release occurred and took some photos. The pump and recirculator package is <br /> still isolated. The facility has a new pump located in the area to be used for <br /> replacement, and the system will be isolated until the issue is corrected. According <br /> to Jeff the release was for about 30 minutes. Incident investigation has already <br /> started and the root cause has not been determined. I discuss the possible cause <br /> of the release with Jeff. I issue a notice to abate, and gave a copy to Jeff. I left the <br /> site at 11:15am and arrived at the office at 11:45am. <br /> Generated the Emergency Response Records, and attached to the report. <br /> Completed the photo log for the inspection photos and attached to the report. <br /> I emailed a copy of the notice to abate to Jeff with the photos I took today. <br /> 12/04/2019 Received the incident investigation report from Jeff Atkinson. I reviewed and PE 2546 SAEED <br /> 60 min replied, "The recommended corrective section is blank. Were there any SC 544 <br /> recommendations/lessons learn from this incident?The report you sent called for <br /> updating Cal OES spill report with the actual quaintly released . Also, the last item <br /> in the notice to abate is to submit the maintenance records (maintaining oil level, <br /> replacing seals, and routine inspection) for the suction accumulator pump. " <br /> I filed the emailed document with the complaint. <br /> At 2:34pm received the updated Cal OES spill report with the amount released <br /> (15lbs). I filled the spill report. <br /> The incident investigation report was updated to include a corrective actions <br /> showing that Pump was replaced. Also, the daily/weekly inspection checklist was <br /> emailed. Since I did not receive records for inspecting the oil level on the pump, <br /> and records showing the last time the seal was replaced. I emailed Jeff asking for <br /> those records. <br /> 12/06/2019 Emailed Jeff Atkinson stating, " Just following up on the corrective actions and PE 2546 SAEED <br /> 12 min email from yesterday. The following element shall be covered during your routine SC 544 <br /> inspection per IIAR 6: <br /> ? Visually inspect for excessive vibration <br /> ? Listen for abnormal sounds <br /> ? Verify oil level in reservoir is acceptable <br /> ? Visually inspect pump assembly for oil leaks and connective tubing for integrity <br /> ? Visually inspect pump bearing wear monitor <br /> ? Visually inspect pump metal surface for pitting or surface damage <br /> ? Verify base frame anchors are in place <br /> If these elements are not followed, please add them as corrective actions .
