Also, if there actually IS a work plan, I will need to have a copy <br /> in the file. Especially important is a site map showing the locations of the borings. Somewhere in this plan should be the <br /> process you're planning on to'destroy these borings when you are done. A cement grout, tremied in to total depth will be <br /> the best and easiest to get approved by EHD. Other methods will require evaluation. <br /> One of the attached forms is the actual permit application. Typically this form is signed by the California C-57 drilling <br /> contractor. But if you sign it instead, then the driller has to sign the bottom of the SECOND form, the LCD, (back page of <br /> the permit).. Either way, the C-57 has to sign something. A$89 check should accompany this application, and LCD, (if <br /> needed). <br /> The THIRD form, the"MFR", identifies the land owner, the facility contact, and who's going to pay the service fees EHD <br /> accrues and is to be sent along with a 3-hour PREPAYMENT check of$267. (@$89 per hour). Typically, the'responsible <br /> party'signs this form. But if you, LS,want to accept our monthly billing (after the intial $267 is exhausted), then fill out the <br /> "Third Party Billing Info"at the bottom of the MFR and send it in with all the other forms, checks, work plan, etc. If you let <br /> me in on your schedule, I'll do my best to meet it. <br /> With approval of the BOARD, properly signed forms, checks, and the work plan (maps, grouting procedure, schedule, etc) <br /> here, I can issue the permit in a day and you can start your field work. Delays occur when forms are missing or not signed <br /> or checks not here, etc. <br /> Once issued, I will call you to advise. We can confirm your dates then... <br /> Although EHD doesn't comment on the work plan usually,we are required to protect groundwater and assure our WELL <br /> STANDARDS Policy. We do this by insuring that the boring,wells, drilling-in-general, doesn't provide a preferrential <br /> pathway for contamination migration. Under normal situations, I will be there for the grouting procedure. You will be <br /> expected to schedule an inspection for this purpose with me.
