" report. These potential conduits were <br /> destroyed without permit or inspection by this regulating agency <br /> (Well Ordinance 11862) , therefore, we cannot rest assured that the <br /> wells were abandoned properly. Specifications for proper <br /> abandonment procedures and permitting requirements were mailed to <br /> Interland Corporation under previous correspondence. Permits to <br /> properly abandon all production wells and monitoring well #3 and <br /> the septic system shall be secured and finaled during the PAR <br /> investigation. (Correspondence of October 3 , 1990, attached. ) <br /> During development and sampling of MW#2 and MW#1, "tap" water was <br /> introduced to aid in the development. While this is suggested as <br /> a last resort, the final phase of development should include <br /> extended pumping until discharge pH, conductivity, and temperature <br /> have stabilized. The sampler, L. Compton, noted on the April 6, <br /> 1990 "Water Sampling Field Survey Form" that the pH and <br /> conductivity readings were not performed due to the absence of <br /> field equipment. <br /> In order to accomplish groundwater monitoring, delineating <br /> horizontal and vertical extent of groundwater contamination, and <br /> verification of the water table gradient and regional aquifer <br /> gradient, a minimum three-point problem solution to define a plane <br /> must be utilized. An additional monitoring well must be installed <br /> downgradient from the tank excavation. Sampling dates shall be <br /> provided to both PHS/EHD and CVRWQCB at least 48 hours in advance <br /> of each activity. <br /> We recommend that during the investigative phase of the PAR, two <br /> consecutive rounds of water quality sampling be conducted. <br /> The PAR will be expected to address the concerns in this letter of <br /> review as well as meet the requirements specified in the "Appendix <br /> A, Tri-Regional Recommendations, Minimum Reporting Requirements, " <br /> and should be submitted to PHS/EHD and CVRWQCB no later than <br /> January 11, 1991. <br />
