kwoo
Memo From the Desk of Mev meavs
To: Stephen Carter, GeoStrategies Senior REHS
Date: February 1, 1996
Subject: Permit Fees
Steve,
Enclosed are two checks for $89: 031360 and 031362. Sorry for any confusion which
PHS/EHD's handling of the checks have caused. The checks are being returned because well
destruction permit fees are $60 each.
I have spoken with Scott Fitchie of Bay Area Exploration and he will forward authorization for
you to sign on his permit applications. Once authorization and permit fees are received I will
issue the permits to destroy wells located at 5431 West Grant Line Road in Banta and at 3246
East Fremont Ave in Stockton.
Also, I have notified Scott Fitchie about the problems which have been associated with the
location of EA5 at the Fremont Ave site. The last report I received from Blaine Tech indicated
that they were unable to locate the well.
