Ms Margaret Lagorio Project No 880-040 IA <br /> November 8, 2002 <br /> Page 3 <br /> In their letter dated October 31, 2002, the SJCEHD requested a work plan to define the <br /> vertical and lateral extent of soil and groundwater impact and determine an interim <br /> remedial alternative to control the off-site migration of MtBE A work plan dated <br /> December 12, 2001 was prepared proposing the implementation of a 5-day DPE event, <br /> and the installation of two downgradient wells and one deep soil boring The work plan J <br /> was approved by the SJCEHD on December 31, 2001 1 <br /> FIELD ACTIVITIES <br /> ' To evaluate the vertical and lateral extent of soil and groundwater impact and an interim <br /> remedial alternative to control the off-site migration of MtBE, Shaw E & I installed two <br /> downgradient monitoring wells, advanced one on-site soil boring, and conducted a 5-day <br /> DPE event <br /> Prior to the initiation of fieldwork, drilling permits were obtained from the SJCEHD <br /> Copies of the permits (429889 and #29891) are included in Attachment A Underground <br /> rService Alert(USA) was notified at least 48 hours prior to the commencement of field <br /> activities <br /> A site-specific Health and Safety Plan was prepared for both phases of work at the site in <br /> general accordance with 29 CFR 1910 120 and Title 8, California Code of Regulations, <br /> Section 5192 the Health and Safety Plan included safety procedures for work to be <br /> performed at the site, chemical hazard information, site safety officers, and preferred <br /> rmedical emergency locations <br /> ' Installation of Two Downgradtient Wells <br /> On May 23, 2002, two monitoring wells (MW-4 and MW-5) were installed at the <br /> locations shown on Figure 2 by Watermark Drilling (C57 #283326) using hollow-stem <br /> auger techniques The two well borings were drilled to a total depth of 25 feet bgs Soil <br /> samples were collected from the well borings at approximately 1 6-meter(5-foot) <br /> ' N lrancfpllProjectslProjects1880104011 a101101Assessmenl and Intenm Remedial Report doc <br />
