The facility <br /> generates used oil at the facility. Please mark yes to question#3 and enter the CAI EPA ID#CAL000302612 in the <br /> designated area. Please update the primary and secondary contact information. <br /> This information must be current and submitted as part of the business plan. This violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> 115 gallons of series 730 liquid catalyzed sulfite, 115 gallons of series 420 boiler treatment, 110 gallons of series <br /> 622 colling water treatment, 250 gallons of glycol, and 3600 pounds of R-22 freon were found on site that have not <br /> been reported. Obsolete federal hazard categories were used on the chemical fact sheets. <br /> Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. This <br /> violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The site map was not completed as part of the business plan.The site map is missing a north orientation, loading <br /> areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log <br /> into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or <br /> updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete.
