'r Quik Stop Market No. 132 Page 2
3555 West Hammer Lane March 1, 2012
Stockton, California 95209
V i
• Wells that have laboratory data from the last sampling event that report non-detectable
` concentrations of contaminants of concern and with acceptable laboratory detection
limits may be destroyed by pressure grouting from the bottom upward, while leaving the
well casing in place. The top three feet or more of the well shall be drilled out with an
auger that is at least one foot larger than the auger originally utilized to install the well,
and the soil around the cut-off casing removed to a depth of an additional six inches
below the new 'top' of the cut-off casing. The grout sealant shall spill out of the top of
the well casing to form a mushroom cap seal, with at least 12 inches of neat cement
. grout poured 6 inches above and 6 inches below the top of the cut-off casing.
• All groundwater displaced to the surface during grout installation shall be pumped out of
the excavation to allow for the EHD inspection and verification that the well has been
properly destroyed. Contaminated groundwater pumped out of the excavation shall be
containerized for proper disposal.
This site has 1 well located off-site in the West Hammer Lane right-of-way. Please
submit a copy of a valid City of Stockton encroachment permit along with the EHD Well
and Boring Permit Application form for this well. Also submit a copy of the access
- agreement for the destruction of two wells located on an adjacent parcel along with the
EHD Well and Boring Permit Application form for these wells.
After the EHD has approved the well destruction scope of work proposal, you may submit an
EHD Well and Boring Permit Application form, Master File Record form, access agreement,
encroachment permit and the appropriate fees for each parcel. The permit fee for the
destruction of each well is $125.00, along with a $375.00 Inspection Activity fee per parcel. A
review of the site records indicates that 9 wells currently are installed. A fee of $2,250.00 (9
monitoring wells x $125.00 + $375.00 x 3 parcels) will be required for the well destruction
permits.
If you have any questions, please contact Vicki McCartney at (209) 468-9852 or by email at
vmccartney@sjcehd.com.
V
Sincerely, /
Victoria L.
