PUBLIC )IEALTH SERVICES {tN
SAN JOAQUIN COUNTY r?° `
ENVIRONMENTAL HEALTH DIVISION
Ernest M. Fujimoto, M. If the well's casing
had not been in such poor condition, a miller's knife might have been used to
perforate the casing. After the well is perforated an approved grout will then be
pumped from the bottom up to complete the destruction.
I have enclosed a copy of the San Joaquin County Well Standards well destruction
section for your information.
Please submit a well destruction permit application, a $60 permit fee and a brief work
plan describing the well destruction procedures which will be used. Once the permit is
issued, PHS/EHD must be contacted at least two working days prior to initiating field
activities so that an inspection can be scheduled.
If you have any questions, please contact me at (209) 468-0337. Thank you for your
cooperation and it is PHS/EHD's hope that the remainder of the construction activities
at the site can proceed more uneventfully.
Donna Heran, REHS, Director
Environmental Health Division
Mary Meays, Se for REHS Margare Lagorio, RHS
Site Mitigation Unit Supervisor
cc: Elizabeth Thayer, Central Valley Regional Water Quality Control Board
cc Brett Hunter, Chevron
A Division of San Joaquin County Health Care Services
