• • Page 2 of 5 <br /> some concerns with your submittal packet: <br /> 1-Our permit application is two pages. The second page, the Licensed Contractor Declaration <br /> (LCD) that contains an area for the C-57 to authorize your signature on page one is NOT here. <br /> I can not issue the permit w/o the C-57's authorization for-you to sign. <br /> See attached WELL-PERMIT-APPL.PDF file <br /> 2-additionally, a map (a single map) is needed that shows all the areas the borings/wells are to <br /> be placed. you have sent in five maps and not one shows all the borings on it. Please <br /> provide one single map, on a 8.5" X 11" page, that shows all 15? 16 borings. also clarify the <br /> correct number of borings please. Plan says 16, your application shows only 15. <br /> The 15 locations are shown on the attached figure Koppel_sample_locations.pdf.There are 15 locations with a <br /> total of 64 borings. Soil sample areas 1K through 7K have 8 borings at each location, with each boring to 6 feet <br /> bgs(total of 56 borings). Locations K11-01 to K11-08 are locations with only 1 boring at each location to 15 feet <br /> bgs (8 borings). So a total of 64 borings. <br /> 3- 'temporary monitoring points' are mentioned in the plan. you need to further explain how <br /> LONG temporary is. ANY and EVERY hole, temp or not, left in the ground AFTER you/your <br /> tech/the driller leaves the site for the night is a WELL. your permit application is not adequate <br /> for wells. as submitted, your permit application is only to be valid for BORINGS....holes that are <br /> destroyed at the end of each day. In order to issue this permit, you will need to clari /call out <br /> that NO boring-will be left open/u ig routed overnight. If you have plans to keep points open <br /> over night, you are to provide a surface seal, a casing securing/locking device and a <br /> mechanically fastened valutl/box. Each well is $122 for permit purposes and will require a plan <br /> for destruction to be approved by the EHD for its destruction. <br /> NO borings will be left open overnight. All borings advanced each day will be grouted prior to the <br /> driller departing site for the evening. <br /> 4 you do not mention how you will destroy/abandonthe holes/monitoringoints. this needs to <br /> be clarified. Materials used are to be discussed in the work plan. <br /> The holes will be grouted to the surface with neat cement. For the 6-foot borings not into groundwater grout <br /> will be poured from the surface, as long as the borings remain open. For the 15-foot borings, which we <br /> anticipate to be in groundwater, will be tremie grouted. For the 15-foot borings the pre-packed screen will be <br /> pulled from the boring and then a tremie sent back down the boring for grouting, as long as the boring remains <br /> open. If borings collapse then the direct push rig will be used to advance rod to the total depth of the boring and <br /> grout back out either through the drill rod or by placing a tremie. <br /> 5- please explain the fee amount you sent in for the permit. BORING permits for one parcel <br /> are $488. ($366 plus $122 for BORINGS). You have sent in $732. Do you have some reason <br /> for the extra $366 being submitted? <br /> When we spoke before you said since boring are on three different parcels that are in the same area that <br /> I need to submit $366 plus $122 for each of the three parcels. The three parcels are outlined on the <br /> attached figure for your reference. <br /> hopefully you can now see why the EHD requires 10 working days to process permits. many <br /> items are typically missing or inaccurate and it takes time for you to forward the correct <br /> materials to us. If I told you July 5 as the latest date for you to submit, this date was offered to <br /> you as the last date for all the correct submittals be here. for future planning, it's ok to provide <br /> more than four working days for the EHD to process your submittals. there is no problem with <br /> the permit being issued months/weeks before the start date. It is valid for one year after <br /> issue. <br /> 7/5/2011 <br />
