(Case No. 2) Page 2 of 2 <br /> 375 West Hazelton Avenue November 24, 2014 <br /> Stockton, CA 95203 <br /> • Containerize rinsate and soil cuttings in properly labeled 55-gallon drums; and <br /> • Notify the EHD at least 48 hours prior to conducting grouting procedures. <br /> The EHD approves the Work Plan with the following conditions: <br /> • During boring abandonment, boreholes shall be filled with neat cement, pumped from <br /> the bottom of the borehole up to at least 3 feet bsg through a tremie pipe, extending to <br /> within 5 feet of the borehole's total depth; <br /> • Notify the EHD at least 2 weeks prior to implementing fieldwork so that an inspector can <br /> schedule to be present for the well destructions; and <br /> • The EHD requires that work may begin no sooner than 5 working days after the EHD <br /> permits have been issued. <br /> To proceed with obtaining the permits for these borings, the following must be submitted to the <br /> EHD to the attention of Gabriel Gonzalez: <br /> • Two EHD Well and Boring Permit applications (one for each parcel/address); <br /> • Master File Record form; <br /> • Licensed Contractor's Declaration (LCD); <br /> • Site access agreement; and <br /> • A permit/inspection fee of $1,040 ($390 x 2 + $130 x 2) to the EHD. The permit fee for <br /> borings per parcel is $130, along with a $390 Inspection Activity fee per parcel. <br /> This EHD approval is limited to only the methods proposed in the work plan for boring <br /> advancement and abandonment, soil and groundwater sampling, equipment decontamination <br /> and waste management.
