2 0 GROUNDWATER MONITORING WELL INSTALLATION <br /> To re-establish groundwater table monitoring capabilities and define the downgradient extent of <br /> impacted groundwater, two additional groundwater monitoring wells were installed <br /> 2.1 Permittingand Planning <br /> 9 <br /> The planning and permitting task included acquiring the required permits and utility clearances <br /> and preparing a site-specific health and safety plan <br /> 2 1 1 Well Permit and Utility Clearance <br /> To perform the proposed scope of work, a well permit (#015434) was obtained from PHSIEHD <br /> (Appendix A) Since both wells were located on the cemetery property, neither encroachment <br /> permits nor off-site private property access agreements were required On April 23, 1998, <br /> Underground Service Alert(USA) was notified to perform a routine utility clearance inspection <br /> (USA Ticket#98949) <br /> 2 1 2 Health and SafetV Plan <br /> Prior to mobilizing for field activities, a site health and safety plan was prepared (Appendix B) <br /> The health and safety plan includes safety procedures for work to be performed, chemical <br /> hazard information, site safety officers, and preferred medical emergency locations <br /> 2.2 Well Installation <br /> On April 28, 1998, V&W Drilling, Inc (V&W) of Rio Vista, California (C-57#720904) installed two <br /> groundwater monitoring wells Monitoring well (MW-6) was installed approximately 60 feet north <br /> of VW-2 and approximately 80 feet north of the former USTs and MW-7 was installed <br /> approximately 80 feet northeast of VW-2 and approximately 100 feet northeast of the former <br /> USTs The locations of the newly-installed wells are shown on Figure 2 <br /> 2 2 1 Well Construction <br /> The soil borings for both wells were drilled to a total depth of 60 feet bgs by a truck-mounted drill <br /> rig equipped with 8-inch diameter hollow stem augers Both wells were constructed with 30 feet <br /> (30 to 60 feet bgs) of two-inch diameter, Schedule 40, flush-threaded and machine-slotted <br /> (0 020-inch) PVC well screen and 30 feet of blank PVC casing to ground surface <br /> 6 <br />
