(Stratus) has prepared this Well Destruction Report, on behalf <br /> of Mr. Dalwinder Dhoot; for Joe's Truck Plaza, located at 15600 South Harlan Road, <br /> Lathrop, California (the Site, see Figure 1 and Figure 2). In a letter dated November 18, <br /> 2009, San Joaquin County Environmental Health Department (SJCEHD) indicated that <br /> the environmental case at the subject property had been approved for closure, and <br /> requested that all groundwater monitoring wells installed at the site, as part of this <br /> environmental case, be properly destroyed. Stratus recently directed the destruction of <br /> the site monitoring wells; activities associated with completion of this well destruction <br /> work are documented in the following subsections of this report. <br /> I <br /> WELL DESTRUCTIONS <br /> Prior to initiating work activities, an encroachment permit was obtained from the City of <br /> Lathrop and well destruction permits were secured from SJCEHD; copies of these <br /> permits are provided in Appendix A. The work areas were marked for Underground <br /> Service Alert (USA) clearance 48 hours in advance, and SJCEHD, the City of Lathrop, <br /> and personnel from Joe's Truck Plaza were notified in advance of initiation of work <br /> activities. The former well locations are depicted on Figure 2. All work was completed <br /> under the direction of a State of California Professional Geologist. <br /> A C-57 licensed drilling contractor (RSI Drilling Inc., C-57 #802334) performed the well <br /> destruction activities, in the presence of a Stratus representative, between February 16 <br /> and 19, 2010. A total of 22 wells (MW-1 through MW-11, MW0-1X, MWO-1, MWO-2, <br /> MWO-3, MW-12C, MW-13A/B, MW-13C, MW-14A, MW-14B, MW-15A, and <br /> MW-1513) were destroyed.
