The approximate upper 3 feet of the well <br /> materials were removed using air knife/drilling equipment, after removal of the traffic <br /> f rated vault boxes covering each well. Following removal of the uppermost portion of the <br /> well, a "mushroom cap" of neat cement was placed over the top of the remaining well <br /> i casing. To allow for placement of the mushroom cap, at least a 6-inch radius of soil <br /> surrounding the well materials (at least one foot larger than the original well borehole <br /> diameter) was removed to a`depth of about 3 feet below ground surface (bgs) to meet <br /> SJCEHD well destruction requirements. After curing of the mushroom cap cement, the <br /> ground surface was subsequently patched to match the surrounding area. <br /> l <br /> Following completion of the work, Stratus submitted well destruction forms to the <br /> California Department of Water Resources (DWR) on behalf of RSI Drilling Company. <br /> Copies of these well destruction forms are included in Appendix B. <br /> WASTE MANAGEMENT <br /> Soil cuttings generated during the well destruction work were contained in properly <br /> labeled, DOT-approved, 55-gallon steel drums and stored on-site pending disposal., A <br /> sample of the soil cuttings was forwarded to a state-certified laboratory for chemical <br /> analysis :in order to determine an appropriate disposal facility.
