ECM will be the third party billing agent.At the <br /> bottom of the MFR form, Binayak Acharya's signature is required as representative of ECM and as <br /> the permit applicant. The form also requires Binayak's Driver License Number and a photocopy of <br /> his license. I do not think this is appropriate since Binayak is not personally responsible for the <br /> costs- ECM is. Is there something else more appropriate I could put instead, such as EOM's tax ID <br /> number?
