0 Page 3 of 3 <br /> Hi Mike:Thank you for all your help yesterday and for directing me to the new County MFR form, <br /> which is now required to accompany the submittal of each permit.This form lets the County know <br /> who to bill if the EHD Inspector's field time exceeds the costs covered by the initial permit fee <br /> ($122 for one hour, plus$366 to process the permit). <br /> I have a specific question for you about that form. ECM will be the third party billing agent.At the <br /> bottom of the MFR form, Binayak Acharya's signature is required as representative of ECM and as <br /> the permit applicant. The form also requires Binayak's Driver License Number and a photocopy of <br /> his license. I do not think this is appropriate since Binayak is not personally responsible for the <br /> costs- ECM is. Is there something else more appropriate I could put instead, such as ECM's tax ID <br /> number?
