We have on file ONE Master File Record <br /> form (MFR) for "Ripon Auto Body, 807 West 5th Street"dated August 17 wherein you added <br /> "FORM INCOMPLETE - FOR EXAMPLE ONLY".... complete forms will be needed before we <br /> can issue. <br /> I understand the applications, Licensed Contractor Declaration (LCD) and additional MFR(s) <br /> will be 'emailed' to the EHD. I will be out of the office Aug 27 thru and including Sept 27. You <br /> may address the emails to my boss, Adrienne Ellsaesser at AellsaesserCaD-sjcehd.com and cc <br /> Frank at Fgirardi sjcehd.com. Chances are Frank Girardi may cover the field work and may <br /> even issue the permits in my absence. Please include copies of the City encroachment permit <br /> (s) and copies of each private parcel access agreements as attachments to your email or send <br /> them by US Mail. The EHD cannot issue your permits w/o these docs. <br /> Joey, is there a formal work plan for this phase? we'll need to verify <br /> well construction/decontruction specs before we issue. <br /> we'll also need a map to issue the permits. How about a CVRWQCB work plan approval <br /> letter?. any immediate questions you have may be addressed by Ms. Ellsaesser at her office <br /> phone #, (209) 468-0343 or Mr. Girardi, at 209-953-7868. Do you have a drill start date in <br /> mind? <br /> AE and Frank, <br /> I've made a notation in the file for this CVRWQCB (29.60) site so you can get up to speed with <br /> the site.
