MONITORING AND REPORTING PROGRAM NO. R5-2014-0815 - 6 - <br /> NESTLE <br /> 6 - <br /> NESTLE USA, INC. -RIPON <br /> SAN JOAQUIN COUNTY <br /> 8. Quarterly electronic reports, which conform to the requirements of the California <br /> Code of Regulations, title 23, division 3, chapter 30, shall be submitted electronically <br /> over the internet to the State Water Board Geotracker database system by <br /> 15 February, 15 May, 15 August and 15 November, until such time as the <br /> Executive Officer determines that the reports are no longer necessary. <br /> 9. Treatment system monitoring data shall be reported monthly. This includes the <br /> existing Industrial Avenue treatment system, the existing wellhead treatment <br /> systems on private supply wells, and any new treatment systems that the Discharger <br /> installs as part of the ongoing remedial actions. The Discharger shall submit <br /> monthly treatment system monitoring reports via email or mail by the 28th day of the <br /> following month. Monthly reports shall include the following minimum information: <br /> (a) A tabulation of all treatment system performance monitoring data, flow rates, <br /> and percent up time. <br /> (b) An explanation of any problems and/or treatment system down time, and <br /> corrective actions taken during the month. <br /> 10.Quarterly paper cop reports shall be submitted to this Central Valley Water Board <br /> office by 15 May (1S Quarter), 15 August (2nd Quarter), and 15 November (3`d <br /> Quarter) until such time as the Executive Officer determines that the reports are no <br /> longer necessary. The 4th Quarter data shall be included in the Annual Report. <br /> Each report shall include the following minimum information: <br /> (a) A cover letter describing the groundwater sampling event and results, how <br /> and when samples were collected. <br /> (b) Groundwater potentiometric surface maps for all groundwater zones (Upper, <br /> Intermediate, Lower);. <br /> (c) A table showing well construction details such as well number, groundwater <br /> zone being monitored, ground surface elevation, reference elevation, <br /> elevation of screen, elevation of filter pack, and elevation of well bottom. <br /> (d) Data tables containing the water quality analytical results and depth to <br /> groundwater for that quarter. <br /> 11.An Annual Report shall be submitted to the Central Valley Water Board by <br /> 31 March of each year. The Discharger shall include data from all four quarterly <br /> sampling events. This report shall contain an evaluation of the effectiveness and <br /> progress of the investigation and remediation. The annual report shall contain the <br /> following minimum information: <br /> (a) both tabular and graphical summaries of all data obtained during the <br /> previous year; <br /> (b) groundwater potentiometric surface maps for each quarter and each aquifer <br />
