• <br /> 3) Follow the guidelines of the EPA Guidelines on Class V Shallow Disposal Wells attachment to close a Class V well <br /> (Case C), but send the reports, etc,to EHD not EPA. <br /> 4) This will be Phase II investigation project initially to see if there is any contamination of the soil and/or ground <br /> water from this injection well.We will require$375 Site Mitigation Work plan fee, Master file record form <br /> completed,the work plan that follows the EPA guidance to sample the bottom and side walls of the <br /> boring/ground water,and the well destroyed by removing all contents of the well by over drilling.A third party <br /> shall collect the samples and deliver to the lab for analysis and the lab shall be instructed to release the results <br /> directly to EHD to my attention.
