Pursuant to the phone <br /> conversation with Ms. Eleanor Ratliff (11-6-91) , these wells are <br /> for "water quality only. " Included with this application is the <br /> required filing fee of $89 . 00 to San Joaquin County Public <br /> Health Services. <br /> The enclosed supporting documentation includes Scope of Work, <br /> RWQCB approval letter, well completion diagram, locations of <br /> wells and maps, and list of constituents to be monitored. In <br /> addition, a letter authorizing us to file the permit application <br /> on behalf of West HazMat Drilling Corporation and West HazMat' s <br /> C-57 form are also included. A copy of West HazMat' s Worker' s <br /> Compensation Insurance form is on file with San Joaquin County. <br /> Mr. David Duncan, factory manager, Holly Sugar Corporation, <br /> Tracy, California, has assured us that the locations of the <br /> eight proposed monitoring wells are greater than 50 feet from <br /> any sanitary sewer line. <br /> We would like to commence well installation activities on <br /> November 14 , 1991.
